Learner's ID Database Submission
PLEASE READ THE INSTRUCTIONS
Step 1: Register Your Account
You must create an account before you can upload your file.
1. Open the Submission Web App portal link.
2. Click Register Here at the bottom of the login box.
3. Create a username and password then click Register.
4. Once your account is created, log in using those credentials.
Step 2: Download the Template Form
Your file must use the exact structure that matches the main database.
1. Open the direct link to the ID DATABASE 2026.
2. Click Copy to save a copy of the spreadsheet to your Google Drive or computer
3. Your template must contain the following headers in Row 1:
No. | Grade Level | Section | Adviser | Name of Student | LRN | Parent/Guardian | Address | Phone
4. Carefully fill out the student records. You may edit or delete information by row as needed.
5. Enter the numbering in sequence (e.g., 1, 2, 3).
6. Enter the Grade Level, Section, and Adviser information.
Step 3: Enter Student Information
1. In the Name of Student column, use the dropdown menu to search for and select a student from the database.
2. Once a student is selected, the available information will be automatically generated and displayed.
3. You may edit, replace, remove, or clear the selected student information as needed.
4. If entering a student manually, use the format: Last Name, First Name, M.I.
5. Enter the student's 12-digit Learner Reference Number (LRN).
6. Complete the Parent/Guardian, Address, and Phone columns.
7. Review all information carefully before saving.
Step 4: Save Your File as a CSV
The portal only accepts .csv files. Standard Excel files (.xlsx) will not be accepted.
Google Sheets:
Click File > Download > Comma Separated Values (.csv).
Microsoft Excel:
Click File > Save As, then change the file type to CSV (Comma delimited) (*.csv).
Step 5: Upload to the Portal
1. Log in to the ID Submission Portal.
2. Drag and drop your saved .csv file into the dotted upload box, or click Browse Files to select it.
3. Click Commit Upload.
Step 6: Check Your Submission
Click the Main Database button in the portal.
Use this to review and verify your submitted entries.
Ensure that all information has been correctly uploaded and saved.
Important Note:
Uploading a new file will automatically delete and replace any student records previously submitted under your account. This ensures that the database remains clean and prevents duplicate entries.
This version clearly separates the student-selection process from the template setup and upload process.
