For Online ENROLLMENT
Parents will be the one to enroll the learner and answer the survey through this online platform.
Prepare the following requirements:
2x2 ID Picture - selfie is not allowed, label your picture with your complete name.
Clear SF9 or School Report Card -(image and pdf format is acceptable, indicate the learner's name on the inside portion of the report card)
Certificate of Good Moral Character (for incoming Grade 7 , 11 and Transferees only)
NSO/PSA - if applicable
