Please bring all medication(s) and the required forms to your learner's campus front office between the hours of 8:00 AM – 3:30 PM before the first day of school and please call the front office in advance if you have any questions regarding this process. Parents who wait until the first day of school will be asked to wait until the school day begins before we will be able to assist them.
The following documentation is required for any over the counter medications that need to be dispensed to your child via the clinic:
·A note written and signed by the child’s PCP on prescription pad or office letter head detailing the following information:
Medication to be dispensed (i.e., Ibuprofen, Midol, Benadryl)
Number of milligrams to take (i.e., 100 mg, 200 mg, 10 mg)
Frequency medication is to be given (i.e., every 4 hrs, every 6 hours)
Time medication is to be given (i.e., as needed, 1:00 PM)
Symptom(s) medication controls (i.e., headache pain, cramps, grass allergy)
