Overview of the Student Central athletic forms.
Sickle cell testing will also be provided for free with a form signed by the athlete's parent or guardian,
REMINDER: The national pediatric association states that anyone in that middle school age range should have their physicals done by their pediatrician. This is regardless of competition level.
The kickoff letters for Student Central registration for students and their parents are below, as well as, the NEW SCHSL Physical Form. All athletic forms will be handled through this new interface. The following is a quick rundown of the steps.
-Students and their parent will need to go online and create a student profile as well as a parent profile. Once this is done, they have to link the two accounts (Basically saying that the student is their kid).
-Once linked, the parent will upload all necessary forms via phone or other device and answer all required questions. If they do not do this, then it will not show up for approval. Therefore, it will not be on file and they will not be allowed to participate in any capacity.
-After a student's forms are approved, then coaches will all be able to have access to see what forms are completed and who has forms in order to allow students to participate in conditioning and tryouts.
