Eligible Dependents include:
Married spouse and civil union partner as defined under the Illinois Religious Freedom and Civil Union Act
Natural children, stepchildren, adopted children (including children in employees' custody under an interim court order of adoption or placed with the employee for adoption or placed with the employee for adoption vesting temporary care) or children for whom the employee is legal guardian under age 26
Dependent(s) who reach the age of 26 will have coverage through the last day of their birth month.
Children who are incapable of self‐sustaining employment and are dependent upon you or other care providers for lifetime care and supervision because of a handicapped condition occurring prior to reaching the limiting age will be covered regardless of age if they were covered prior to reaching the limiting age stated above. Complete a Disabled Dependent Certification, click here and forward it to Blue Cross Blue Shield.
Unmarried children to age 30 residing within Illinois, having served as an active or reserve member of the Armed Forces and released or discharged with other than a dishonorable discharge
This coverage does not include benefits for grandchildren (unless such children have been legally adopted or are under your legal guardianship). Refer to “Family Coverage” provided in the Certificate of Coverage for your insurance plan.
