STEP 1:
Stakeholder accomplishes the Facility/Equipment Use Form (UREC -QF-28);
STEP 2:
Stakeholder consults with the laboratory head or staff regarding the availability of their requested equipment.
STEP 3:
Stakeholder submits the following requirements to the TSD at least seven days prior to their use:
a) One copy of duly accomplished Facility/Equipment Use Form (UREC -QF-28); and
b) One copy of protocol approved by their respective college/unit research committee.
STEP 4:
Stakeholder waits for the notification regarding the status of their request
STEP 5:
If approved, stakeholder attends orientation from the laboratory staff on the laboratory safety rules and regulations and standard operating procedures on the use of chemicals, apparatuses and equipment including the schedule of laboratory fees.
STEP 6:
Should the stakeholder requests to work outside the official hours of the university, the stakeholder notifies TSD by submitting accomplished Request to Use Facility/Equipment Outside Official Hours (UREC-QF-32) at least one working day before the requested date.
STEP 7:
After the stakeholder submits of required forms and has been cleared all of the accountabilities, stakeholder accomplishes Facility/Equipment Use Clearance Certificate (UREC-QF-33) for their clearance.
