New Student Registration -Next Steps

Thank you for submitting the New Student Registration form for your child. Please read below to determine what documentation you will need to bring to your registration appointment.

Once you have gathered ALL required documents, please call:

Information needed for the Registration Appointment

Student Residential Information

This section is used to gather information as to whether a student lives within the Cuyahoga Heights School District boundaries or is a Non-Resident (Tuition) student.

  • Incoming "resident" families will be asked to provide Proof of Residency as described in the section below.
  • Incoming "tuition" families will be asked to pay a $200 deposit at the time of their acceptance.

Proof of Residency Information

All new resident students entering the Cuyahoga Heights School District must provide the proper documentation to establish proof of residency. The name and address on these documents must match the name and address of the custodial parent or legal guardian of the student(s) being registered. Acceptable proof-of-residency documents are:

Homeowner

(one of the following)

  • A copy of your most recent property tax statement
  • A copy of the settlement statement.
  • A copy of the warranty deed

and

  • 2 Utility bills (example: gas, electric, telephone, cable)


Recently closed on a new home

  • A copy of the settlement statement from closing or a warranty deed

and

  • 2 Utility bills (example: gas, electric, telephone, cable) as soon as they become available


Recently made an offer on a home

  • Signed Purchase Agreement

and

  • Upon closing, provide a copy of the settlement statement or a warranty deed within 60 days.
  • 2 Utility bills (example: gas, electric, telephone, cable) as soon as they become available


Renter

  • A copy of your lease agreement with the term listed. The agreement MUST contain property owner’s name, address, phone number, and signature; and name and signature of parent/guardian.

*Note: Moving in with a person in an apartment will not be accepted, unless your name is on the lease or listed on the lease under occupants.

and

  • 2 Utility bills (example: gas, electric, telephone, cable)

*You will be required to re-submit updated copies of the above information at the end of each lease agreement term for the duration of you being a renter


Renter on a month-to-month basis

  • Expired, previously signed Lease, showing language that after expiration of the Lease said Renter begins a month-to-month contract. Must include landlords phone number.

and

  • 2 Utility bills (example: gas, electric, telephone, cable)


Co-Resident (living with family or friends)

  • Sworn Statement of Residency needs to be completed and NOTARIZED.
  • The most recent property tax statement of the person that owns the home that you are living with is required. This is valid for only one (1) year.

* Note: Moving in with a person in an apartment will not be accepted, unless your name is on the lease or listed on the lease under occupant.

and

(2 of the following displaying your name and current Cuyahoga Heights address.)

  • Utility bill (example: gas, electric, telephone, cable)
  • Work records (pay stubs)
  • Public assistance (example: check stubs, forms)
  • Driver's license
  • State identification
  • Voter registration


Non-Resident Information (Tuition)

Once approved as a tuition family, you will be asked to submit a non-refundable $200 deposit. This deposit will be applied towards your first tuition installment and will be due within 2 weeks of your approval. Payments must be made at the Treasurer's Office located in the High School at 4820 East 71st Street, Cuyahoga Heights, OH 44125. Only cash or cashier's checks can be accepted for the deposit.

*As an established tuition family, you will then have the ability to pay online by credit card for future installments.

Student Background

This section is used to gather information about a student's previous academic history.

Any of the following documents that pertain to the student will be required prior to admission:

  • Report cards
  • State testing results
  • Gifted results
  • IEP
  • MFE or ETR
  • 504 plan
  • Kindergarten Readiness Assessment (KRA)


Custody/Guardianship Information

*Only the person who is the residential parent, legal custodian, legal guardian, or, in limited circumstances, a grandparent of the child may register the child.

If there are court ordered restrictions regarding the custody of the child, you will need to bring supporting documentation (outlined below) to your registration appointment.


Parents with No Custody Restrictions

  • Driver's Licence or State I.D. of each parent

and

  • Student's original Birth Certificate (we will make a copy)


Joint or Sole Custody

(one of the following)

  • Certified time-stamped complete Judgement Entry Divorce Decree, including All Attachments and any Modifications.
  • Time-stamped notice of the appointment of guardianship from Probate Court.
  • Certified Journal Entry designating custody and the school district responsible for educating.

and

  • Driver's License or State I.D. of all parents having custody

and

  • Student's original Birth Certificate (we will make a copy)


Foster Parent, Legal Guardian or Legal Custodian

  • Court Placement Papers

and

  • Driver's License or State I.D. of foster parent, legal guardian or legal custodian

and

  • Student's original Birth Certificate (we will make a copy)


Grandparent Power of Attorney or Caretaker Authorization

  • Grandparent power of attorney or caretaker Authorization Affidavit

and

  • Driver's License or State I.D. of foster parent, legal guardian or legal custodian

and

  • Student's original Birth Certificate (we will make a copy)


Contacts

At the time of initial, online registration, "contacts" can be designated or given the following permissions by the Legal Guardian:

  • Legal Guardian
  • Lives with Student
  • Authorized to Pick Up
  • Medical Contact

*If at any time you wish to edit/add contacts, you must contact the building secretary to do so. You will only have a chance to edit contacts yourself once a year when the online form is open for you to update student contact information.

Medical

  • Immunization records are required for every incoming student
  • A Physical Exam is required for every incoming Pre-Kindergarten student and must have been performed within the last 12 months. Should that exam expiration date occur within the school year. another exam will be required.
  • Students in Pre-Kindergarten must have a doctor and dentist.
