Hello! I am very excited to join the AAHS team! I am blessed to be among students who are interested in healthcare fields. I am looking forward to helping our students promote and maintain good health. Additionally, with experiences as an EMT, medical assistant, dialysis technician, pediatric nurse, school nurse, and nurse practitioner, I hope to be a source of information for students exploring their professional interests.
I live with my husband, 2 daughters, and our dogs. I enjoy exploring national parks, hiking, biking, reading, and making desserts.

EXCLUSION DUE TO ILLNESS
Upper Respiratory Illness and Influenza
Students may return when they are fever free for 24 hours without the use of fever reducing medicines AND symptoms are improving.
Strep Throat
Students may return after 12-24 hours of antibiotics AND fever free for 24 hours without the use of fever reducing medicines.
COVID-19 Infection
Students testing positive for COVID-19 infection may return when they are fever free for 24 hours without the use of fever reducing medicines AND symptoms are improving.
Students should wear a mask when around people indoors for the following 5 days.
Fever
Students with a temperature of 100.0 degrees or higher may return when they are fever-free for 24 hours without the use of medication
Suspicious Rashes
Students with rashes that are suspicious for communicable disease may return after 24 hours with a doctors note indicating treatment is in progress. Non-contagious, weeping rashes must be covered.
Vomiting/Diarrhea
Students with vomiting OR diarrhea due to illness may return when they are symptom free for 24 hours.
Red Eyes
Students with redness and purulent discharge may return after 24 hours with a doctors note indicating treatment is in progress or when eyes are clear.

Medical Summary Form
Submit this form at the start of each school year even if your child is attending remote instruction only. Click on the picture to access the form.
Ensuring that your child's medical information is current will help provide appropriate care for your child.
CONFIDENTIALITY - your child's health information will be shared with other educational staff and emergency care personnel on an as needed basis for health, safety and educational purposes. If there is any information that you would like kept confidential with the nurse only, please indicate that on the form next to the information.
Vaccination Records are only needed for entering freshman.
MEDICATIONS & OVER-THE-COUNTER PRODUCTS
STUDENTS MAY ONLY CARRY LIFE-SAVING MEDICATIONS (with doctors orders)
Only orders from physicians, nurse practitioners, or physician assistants are acceptable.
Orders must be submitted for each new school year.
See below for the applicable forms for each medical condition or product. All pages must be completed and returned to the school nurse prior to the start of the school year.

Medications Administered on Overnight Field Trips
This form is required in addition to all other forms when any medication or over-the-counter product will be needed during a field trip.

MISCELLANEOUS MEDICAL NOTES
Only notes from physicians, nurse practitioners, or physician assistants are acceptable.
Fitness Class Excuses
To be excused from fitness class, students need a medical note. If the note does not indicate a specific end date, a separate medical clearance note is required to resume participation.
Medical Devices & Ice
Medical notes are required for devices such as slings, braces, splints, crutches, and wheelchair use. Additionally, a medical note is required for continued application of ice to an injury beyond 24 hours.
Temporary Accommodations
Temporary accommodations due to injury or illness require a medical note indicating the specific accommodations as well as duration.
504 Documentation
Any student with a 504 must submit an updated medical note indicating the diagnosis and necessary accommodations for each new school year.

Screening Schedule
Height, weight, & blood pressure - yearly for all grades
Vision - 10th grade and as needed
Hearing - 11th grade and as needed
Scoliosis - 9th and 11th grades
Important Information
Please contact me if you have any concerns regarding screenings. Screenings may be declined or adjusted.

submitting a copy of your home district sports participation exam, or
submitting a note in writing
