COMMENT CARD WITH PARENT INFO
Use this card when you want to gather a lot of info from your students. Change or delete whatever fields you don't want.
OUTREACH COMMENT CARD
This card has just enough information on it for a student to quickly fill it out and hand it in. Instead of having a space for "I received Christ at this meeting", you can say, "If you asked Christ to come into your life today, put an X in that box"

Good all-around comment card for any occasion. Notice the places on the bottom where you can put your social media channels. Please edit those before you print!
