Washington DC Itinerary/Registration/Payment Information

FORMS (These forms are fillable using your child's ChromeBook):

Eligibility Agreement - Please complete by October 7, 2025 even if your child does not plan to go to DC.

Overnight Field Trip Form - Please complete by October 7, 2025.  If there are changes we can manually adjust them.

Paper Forms (You must download and fill these forms out by hand and return them to Stacy Samarigan in the Gold Office)  These forms are NOT Due until early March!

Non Prescription Medication Form - All non prescription medication require that this form be filled out and the non prescription medications should be turned in to the nurse in its original container.  

Prescription Medication Form - All prescription medications require 1 form per prescription.  If it is already on file at school, such as an inhaler or Epipen, you don't have to complete the form.
