Final Forms - Signing Documents

Final Forms is the online document signing tool that Columbus Academy offices use for contracts and other school event-related forms. 

To access the Final Forms site, you need to first understand that your family's Final Forms account login is registered to the same email address used to communicate and register your child to Columbus Academy. Students should use their own CA email account to sign in.

1) To get started, you need to first access the Final Forms site. You can either do so via the direct links provided above, or through Columbus Academy's MySchoolApp portal -> under the 'Resources' section:

Or you can try to access it from the CA Parents' web site, under the "Health and Wellness" section:

2) Families should click on the "Parent" login button, and CA students should click on the "Students" login button, respectively:

3) FAMILIES: If you have been using a Google email account to communicate with CA administration, then you can use the "Login with Google" option. If you have been using any other email address you can use the "Never logged in" link to request for a new password to sign into Final Forms:

STUDENTS: Select "Login with Google". The reason is because all ColumbusAcademy.org email accounts are hosted by Google, and so they are actually Google accounts with Columbus Academy branding:

4) FAMILIES ONLY: If you are in the latter category for logins, to get the new password, simply enter the non-Google email address into text input field provided and look for the email to receive your new password to sign into Final Forms.

STUDENTS: You will be taken to the Google login prompt - proceed to sign in with your CA email address. New CA students should use their password provided in their families' MySchoolApp "Files" to sign in (and you may need to go thru a few steps to activate your new Google Account):

NOTE: If you encounter a Final Forms error such as this while attempting to use "Login with Google" (Google SSO) instead of getting redirected to the Google login page, likely it is because you are already signed into a Google Account that is not recognized by Columbus Academy on that same web browser, so please try:

5) Once you're signed in, you can start working on your child's (or the CA students' own) required forms in the Final Forms web site. Here is a summarized overview of the main user interface on the Final Forms home page:
