The Homeless Prevention team helps to prevent and address homelessness for families and individuals in rural Polk County. 

This includes: 

• Helping maintain and find new housing

• Getting connected with mental health & addiction services

• Assistance with basic immediate needs such as providing snack bags and food boxes for families 

• Meeting with families and individuals 

• Conducting street outreach

• Connecting with community partners

FCO partners with programs to assist those who are in need of support to transition from being in need to becoming self-sufficient.
