School Counseling Consent Forms

All students have access to school counseling services to assist them with problems or concerns relating to academic, social/emotional, and career development topics. The purpose of these services is to address the barriers that impact learning.

Individual and group counseling services require written parental consent when services are provided on an ongoing basis, after three sessions. Sporadic, one-time, topic specific situations do not require written consent because they are not considered to be ongoing services.
