PHYSICAL INFORMATION

Students must have a physical completed by a physician before they can participate in athletics in Clayton County.

The physical must be completed on the approved physical form (located below).

The physical must have the physician's signature, date, and stamp.

We understand that it may be difficult to get a physical before tryouts begin. Unfortunately, we cannot allow any physical participation without a physical. It will be up to the coach if they choose to allow a student to tryout after the official tryout date.

If you are having trouble making an appointment with your family doctor, the following offices can usually do an appointment for a physical fairly quick:
