Do I qualify?

Health First Colorado (Colorado’s Medicaid program) is free or low-cost health coverage for Colorado children and adults who qualify. The Colorado Department of Health Care Policy and Financing (HCPF) oversees Health First Colorado and Child Health Plan Plus.

Check to see if you qualify through the PEAK application. Most people find out right away if they qualify.  

Apply by phone: Call 1-800-221-3943 / State Relay: 711. Phone applications are available Monday through Friday from 8 a.m. to 4 p.m.

Apply by mail: Download and print a paper application.

Apply in person: Apply in person at your county of residence’s local county office or at a local application assistance site.

Child Health Plan Plus (CHP+) is public low-cost health insurance for certain children and pregnant women. It is for people who earn too much to qualify for Health First Colorado (Colorado's Medicaid program), but not enough to pay for private health insurance. 

Who Qualifies?

• Children age 18 and under and pregnant people

• Applicants with household income under 260% of the Federal Poverty Level (FPL). See the income table below for more details.

• Colorado Residents

• Applicants not eligible for Health First Colorado

• Applicants who do not have other health insurance
