The Department of Labor is very clear on this subject and requires all businesses that use meat slicers to strictly monitor and control access to these dangerous machines. You are doing an honorable service by reporting violations. In fact, you may even prevent someone from being badly injured!

We feel very strongly that good behavior should be rewarded! That's why we will pay $50.00 cash for notifying us of any violations.

2. Date of violation(s).

3. Time of violation(s).

4. Name of the under age person using or cleaning the slicer or any of its parts.

5. Your name (to claim your reward).

Please note...This also applies to anyone washing the parts in the dish area!

We will immediately review the cameras to confirm your report and your cash reward will be deposited with your next paycheck! It's that easy!
