Welcome to Jay Stream Middle School Athletics and Clubs!

We are excited to have you participate in our extracurricular programs this year at Jay Stream Middle School. We offer a wide variety of clubs, activities, and athletic programs, and we strongly encourage that all of our students get involved. As part of our extracurricular programs, we hope to prepare our students with the tools necessary to be contributing members of our school, local, and global communities. Students will learn the benefits of teamwork, collaboration, diverse perspectives, and camaraderie as a member of our extracurricular programs.

In order to participate in a sport at Jay Stream you must have the following three things:

Parent Commitment Form completed prior to practice or tryout

Athletic Physical submitted to the school nurse prior to practice or tryout

Athletic Payment

Non Cut Sports

Payment due prior to the first practice

Cut Sports

Payment due by the end of the first week's practice

Parent Commitment Form

This form must be completed prior to a student athlete trying out or joining a sport. Students must have a current physical on file. Athletic physicals expire after 13 months. It is recommended that students get their physicals in the summer to span the course of the school year.

Payment due if a student participates in a sport or in a club. (Cash or Check is also accepted for payment in the Jay Stream office)

Illinois Prairie Athletic Conference Code of Conduct

The Illinois Prairie Athletic Conference (IPAC) and its member schools have made a commitment to good sportsmanship by athletes, coaches, and spectators at all IPAC sanctioned events. We require your cooperation through the following guidelines:

Respectful Conduct: All attendees – including parents, visitors, and students – are expected to conduct themselves respectfully to ensure a positive atmosphere for everyone.
Support Our Officials: Show respect for the decisions and authority of our game officials regardless of your opinion of a call.
Trust Our Coaches: Support our coaches' leadership by not coaching from the sidelines.
Be Positive: We value enthusiastic and appropriate cheering that supports players and teams. Negative or demeaning cheers are not permitted.

Failure to adhere to these policies of good sportsmanship may result in a warning and/or removal from the event site to ensure a positive environment for all participants and spectators.

We value your partnership in upholding these standards, as they directly benefit all our athletes and the integrity of our competitions within the Illinois Prairie Athletic Conference. Thank you for your cooperation in ensuring a sportsmanlike and enjoyable season for everyone.
