If you register correctly, you will get an email response for each appointment you make.

APPOINTMENTS are NEEDED - MUST PRE-REGISTER BELOW
Due to space limitations, we ask that you please remain outside the doors until your scheduled time. A member of the staff will check you in, double-check your completed form, and then move you inside.
As time slots fill, they will be removed as an option.
Please do not show up as we are setting up because we will have to turn you away until later.
Only ONE appointment per child. Do not book multiple appointments for the same child or one will be removed.
By registering online, you are granting permission for your child to participate in the free physicals offered by ChristianaCare Union Hospital/Cecil County Public Schools.

Reminders:
Yes, you need an appointment. You will receive an email from Google Forms with your submitted responses. If you DO NOT GET THAT EMAIL, there was an error and you must go back again and be sure to hit the SUBMIT button.
Please arrive ON TIME for your appointment.
Please wait outside the doors until you are "checked in" by a member of the staff.
Come dressed in shorts and a T-shirt.
Athletes: Please have your FULLY COMPLETED Athletic Form Packet with the Medical History ready to go and signed. These will be collected at the end of the evening by the Athletic Directors and kept for the next school year for you.
School of Tech & Private School students only need to have page 1 (medical history) completed & signed; nothing else in the packet pertains to you and is not required by CCPS. Copies of your physical will not be collected or retained.

Gr. 6-12 Guidelines Packet (Please read and maintain for your personal reference.)
Gr. 6-12 Athletic Forms Packet (All items need to be signed and completed. The Medical History form in the packet must be completed before the physical.)
