ChristianaCare Union Hospital, in partnership with CCPS, is offering free physicals for the NEXT school year.

If you register correctly, you will get an email response for each appointment you make.
Be sure to print out or save it so you can show that as proof of your appointment if needed. You must hit the submit button after each registration.

Registration Status: Registration opens on May 20, 2027.
Waitlist: As time slots fill, they will disappear from the system. If appointments are canceled, those spots will immediately reopen online.
Only OUR SPORTS FORM may be used.
If you need anything else, please make sure that the other organization will accept our form. That means you would go to our event with OUR FORM ONLY. We will NOT complete external forms. If you need documentation for another group, please verify that they will accept our form.
By registering online, you are granting permission for your child to participate in the free physicals offered by ChristianaCare Union Hospital/Cecil County Public Schools.

Reminders:
Yes, you need an appointment. You will receive an email from Google Forms with your submitted responses. If you DO NOT GET THAT EMAIL, there was an error and you must go back again/ Be sure to hit the SUBMIT button. If there are duplicate bookings for the same child, only the LATEST submission will be kept.
Please arrive ON TIME for your appointment.
Please wait outside the doors until you are "checked in" by a member of the staff.
Come dressed in shorts and a T-shirt.
Athletes: Please have your FULLY COMPLETED Athletic Form Packet with the Medical History ready to go and signed. These may be collected at the end of the evening by the Athletic Directors and kept for the next school year for you. Be sure to use the forms for the next school year, 2027-28.
School of Tech & Private School students only need to have pages 2 and 3 (medical history) completed & signed; nothing else in the packet pertains to you and is not required by CCPS. Copies of your physical will not be collected or retained.
