Eligibility

ELIGIBILTY DOCUMENTS:

Sports Physical: must be completed by parent (pages 1-2) and doctor (pages 3-4) and be current (1 year) 

- please complete and return ALL 4 pages

Athletic Packet: completed and signed by parents and athlete (ALL FORMS including the GHSA  High School Forms)
