Assessment of Care Survey 

If you had any appointment at Achieve on Monday April 27 - Friday May 1 or Monday May 4 –Friday May 8, complete the anonymous and confidential Assessment of Care Survey at the link below!  It’s easy! Read below to see if you are eligible. If you are, click the link and complete the survey, one survey per person.  Last day to complete the survey is Friday May 8. Note: Not for CFTSS only clients.  

Survey for ADULTS (19 years old and up):  

1. You must have been seen in the clinic since at least January 27, 2026.

Survey for CHILDREN (0-18 years old - To be completed by a parent or legal guardian.):  

1. The child has been seen since at least March 27, 2026, one survey per child seen.

Survey for YOUTH  (12-18 years old): 

1. To be completed by a youth.
