Assessment of Care Survey 

If you had any appointment at Achieve on Sunday May 11 - Thursday May 15 or Sunday May 18 –Thursday May 22, complete the anonymous and confidential Assessment of Care Survey at the link below!  It’s easy! Read below to see if you are eligible. If you are, click the link and complete the survey, one survey per person.  Last day to complete the survey is Friday May 10. Note: Not for CFTSS only clients.  

Survey for ADULTS (19 years old and up):  

1. You must have been seen in the clinic since at least February 11, 2025.

Survey for CHILDREN (0-18 years old - To be completed by a parent or legal guardian.):  

1. The child has been seen since at least April 11, 2025, one survey per child seen.

Survey for YOUTH  (12-18 years old): 

1. To be completed by a youth. 

2. You have been seen since at least April 11, 2025.
