MID-YEAR CHANGES
CHANGING YOUR BENEFIT ELECTIONS
For all pre-tax and some post-tax benefit options, team members are only permitted to make mid-year elections changes under certain circumstances. A qualifying life event (including but not limited to: marriage, divorce, birth of a child, gain or loss of other coverage, etc.) is required to add/remove coverage and/or dependents.
If you are wanting to make a change mid-year and you are not sure if you are eligible to do so, please always feel free to reach out to your HR representative or benefits@burrislogistics.com for questions.
UPDATING YOUR BENEFICIARIES
Team members can update their beneficiaries at any time throughout the year and should review theri beneficiary elections no less than annually to ensure it aligns with their wishes.
Burris team members that meet the eligibility requirements will be automatically enrolled in the 2023 Wellness Program.
Eligibility Requirements:
Actively employed on 12/01/2022,
Enrolled in the medical plan as of 01/01/2023,
and have a hire date or adjusted hire date that is on or before 07/01/2022
Eligible team members must register on the b.Well platform to participate and avoid a surcharge in 2024.
*Program eligibility is lost if the team member is separated or drops medical coverage during the year and cannot be re-enrolled until the next calendar year..
WELLNESS VISIT REQUIREMENT
All eligible team members must be registered on the b.Well plat form AND complete their annual wellness visit and biometrics no later than September 1, 2023 to avoid a $10 weekly surcharge (totaling $520 annually!) in 2024.
In addition to avoiding the surcharge, team members with completed wellness visits will also earn a $50 reward!
NOTE for Dependents who are employed by Burris Logistics:
Team Members covered as dependents are eligible to participate in the Wellness Program. This includes being able to register for b.Well, complete quarterly challenges for gift card rewards, and earn the $50 reward for completing their wellness visit.
For any employed dependent that does not complete their wellness visit by the deadline, the primary subscriber will have the surcharge deduction applied in 2024.

The 2023 Wellness Program will be available in the b.Well app starting January 17, 2023.

Complete your Health Portrait & Medical History for a $50 reward and 600 points to use in the app!
Quarter 2 - Healthy Habits | Work-Life Balance:
Complete 5 in-app challenges related to work-life balance. Earn$25 for completing all 5!
Quarter 3 - Healthy Habits | Stress Less:
Complete 5 in-app challenges related to managing stress. Earn$25 for completing all 5!
Quarter 4 - Healthy Habits | Healthy Choices:
Complete 5 in-app challenges related to making healthy choices, including setting goals, physical activity, etc. Earn$25 for completing all 5!
