Blessed Sacrament Chromebook Lease Agreement 5th - 7th Grade ONLY

Payments for the lease-to-own program can be made through this site. You can also access this payment site using the 1:1 Chromebook Program website or through the Blessed Sacrament website > Enrollment > Registration > then scroll down and click on the school fees link. You may also bring a check to the school office before August 12th, 2025.
Please keep a copy of this form for your records. You can see the Blessed Sacrament School office to obtain a copy of your signed lease agreement or print a copy from the 1:1 Chromebook Program website.

TERMS AND CONDITIONS:
Blessed Sacrament School is initiating a lease-to-own program for students to lease a Chromebook laptop while actively enrolled at Blessed Sacrament School. The lease includes the Chromebook device, charger, 5-year warranty, hardcase, and a soft carrying case. Students will receive their chromebook starting in 5th grade and the device will remain theirs until they graduate from Blessed Sacrament School. Upon graduation, they will own the Chromebook once the lease is paid in full.
I understand that I will be responsible for the payment of the 1st lease agreement of $125.00 (5th and 6th graders) or $65 (7th graders) paid in full by August 12th, 2025. My student will not receive their Chromebook until the lease payment is received. (There is a $10 upcharge if paid online by credit card)
My student will receive their Chromebook the first week of school and the device will remain the property of Blessed Sacrament School until graduation. During the terms of the lease, if a student is not enrolled at Blessed Sacrament School for any reason, the lease will be terminated, or you can opt to buy-out the lease and keep the device.
CARE FOR CHROMEBOOK
Each student is responsible for keeping the Chromebook and accessories in good working condition. If repairs are necessary, the Chromebook must be turned in to the Technology Coordinator to determine if repairs can be made on-site or if it will need to be sent away for warranty repair. If a device needs to be sent away, a student will receive another device on loan until their device is returned. If a device has been damaged beyond repair due to student misuse or failure to take responsible care of their Chromebook, you are responsible for the total replacement cost of the device. This also goes for the hardshell case and soft zipper case. If any damage or defacing occurs to the hardshell case, replacement cost will be $20 for the hardshell case and $8 for the soft zipper case. Students are responsible for the handling of their Chromebooks when traveling within the building. Chromebooks must have the hard shell protective case on at all times, and when traveling, the Chromebook must always be stored in the protective sleeve and zipped. Chromebooks should never be stored inside of backpacks. Violations of these rules will result in an automatic Responsibility Referral.
LOANING OR BORROWING CHROMEBOOK
The student SHALL NOT loan his/her Chromebook to another student or borrow a Chromebook from another student. Each student is responsible for his/her Chromebook and any damages that may occur while in their possession.
MODIFICATION OF THE CHROMEBOOK
The student shall not modify the Chromebook in a way that will permanently alter it, either physically and/or electronically, other than as instructed by an administrator or other school personnel. The student is not permitted to apply marks, stickers, or other decorations to his or her assigned Chromebook. The Chromebook will be labeled by Blessed Sacrament School to identify and track each device and this label may not be removed.
SOFTWARE AND APPLICATION
Blessed Sacrament School reserves the right to audit and remove any software in the student assigned device at any time. Blessed Sacrament Technology Department may require the installation of software that will have access to personal information stored in the student assigned device.
DATA SECURITY/INTELLECTUAL PROPERTY/ ACADEMIC INTEGRITY
Students are responsible for the security of the data stored on his or her school device. No Passwords or login information should be shared with any classmate. The student recognizes that all content created on a Blessed Sacrament School device and using a school Google account is not subject to intellectual property claims. The student further recognizes that all activity should follow the school rules for Academic Integrity.
