Road Map to Preschool Registration

Step #1 Screenings:

Eligible students ages 3-5 must participate in a screening prior to enrollment. Screenings take place in the months of January, June, and August by appointment. To schedule a screening please contact the preschool office at 508-977-0504 ext. 100. Once your child has been screened please follow the directions below to complete the registration process.

STEP #2: Complete the online registration application

Once you have collected all the required documents (see below), please click here to complete the online application and upload your documents. You will receive an email confirming that the application and uploaded documents have been received.

STEP #3: Collect required documents (see below) and upload:

  • Original Birth Certificate with a raised seal

Proof of residency 1: (one of the following)

  • Quitclaim deed (signed & dated)

  • Property card (available on your town’s website – assessor’s database)

  • Lease (signed by all parties & dated)

  • Notarized Letter (if applicable)

Proof of residency 2:

  • Mass. Driver's License or Mass. Identification card with current address
    (to update your address go to and print out a copy of the address change)

  • Proof of up-to-date immunizations, physical exam, and lead test

  • Custody Documents (if applicable)

STEP #4: Review and Submit your Registration
