A consent form, doctor's physical, and concussion form. All of these can be filled out on AKTIVATE - ATHLETE REGISTRATION.

All 7th graders need a physical, performed by a doctor, before they can participate in any school sports. This is different from the PE consent sometimes filled out to give permission to participate in the Physical Education class.

Answer: You can choose to play on Heavyweights even if you are under the 115 pound cut off. You cannot choose to play on Lightweights if you are over 115 pounds. This is done for safety reasons and is non negotiable.

You can wear your own practice Jersey, however, it is recommended to get one from Les Bois that you can use for 7-8th grade football.

If you have a helment, shoulder pads, or some other personal equiptment that you prefer to use instead of the Les Bois gear you need to have this equiptment checked out by a coach to make sure it is safe and useable.
