• All incoming 7th-grade students are required to complete a health physical after May 1st of this year, before entering 7th grade. This physical fulfills the requirement for PE and is also necessary if your student plans to participate in school sports. This can be completed with your primary doctor or at urgent care facilities.

• If your student participates in sports, additional physicals will be required again in 9th grade and 11th grade.

• Please note: We will not accept paper copies of health physicals. All forms must be uploaded to a website called Aktivate. For more information, click “More on Aktivate” below. The link will provide instructions on creating an account for your family.

As parents or guardians, filling out the Student Health History Form gives the health office permission to give:              

 Cough Drops   Ibuprofen   Tylenol         Tums

Without this form in our records, we cannot give students medication. This form is completed each year for any students attending a BSD school.  

***Note: We do not have cold medicines, cough medications, allergy medications, bottled water, or anti-nausea medications available in the Health Office. 

**Other medications: students are not to carry any medications during the school day and all medications must be stored in the health office. Please contact the health office to start completing the paperwork needed to do so.
