Medication Policy

In an effort to keep all students safe, all medication must be delivered to the nurse's office by a parent/guardian. Students are NOT permitted to have ANY medication in their possession (unless they have been given medical approval).

Students who require medication, either prescription or non-prescription, during the school day or during school related activities, must meet the following district policies on a yearly basis:

1. Written authorization from the prescribing physician must be on file in the nurse's office. This should include student's name, birth date, medication, dose, route, time, date, and prescriber's signature, title, address and phone number.

2. Written authorization from the parent/guardian must be on file in the nurse's office.

3. Prescription medication must be in the original pharmacy container, labeled with the student's name, medication dose, route and time. Non-prescription medication must be in the original container with student's name attached.

4. An additional empty prescription bottle, labeled by your pharmacy with your child's prescription is required. New York State has directed school nurses to send this type of container on field trips with proper medication dosage. Your pharmacy will be able to provide this to you. 

NOTE: All medication must be delivered to the nurse's office (or school office) by the parent/guardian. Students are not permitted to have ANY medication in their possession (unless they have been given medical approval). This includes prescription medications and "over-the-counter" medications (such as cough drops, Tylenol, Advil).

All medication will be stored in the nurse's office. All medication not picked up by parents by the end of the school year will be discarded during the summer.

A system has been instituted in the nurse's office in order to assure proper and accurate medication delivery. This includes student photo identification, and medication sheets that the nurse and students are required to sign when medication is given.
