What to Pack: Packing List

Required paperwork for ALL students by Tuesday, September 24th:

1. Permission Slip (Online Mach Form)

2. Emergency Medical Form and OTC Medication Form (Online Mach Form)

3. All payments due (total cost $155)

Required only if taking prescription medication:

1. Due Tuesday, September 24th: Prescription Medication (Paper Copy completed by doctor)

2. Due Tuesday, October 1st: All medications due in original containers marked with your child’s name (and prior medical authorization form submitted)

No medications will be collected the day students leave for camp, unless otherwise arranged with the school nurse AND principal.
