Each month, as Beta Members, it is YOUR responsibility to complete this form to record your service hours.

Service hour requirements:

• 7th Graders: 10 Hours by May ( Submit at least one hour per month). You are encouraged to complete more than the minimum of 10 hours.)
• 8th graders: 15 Hours by May (Submit at least one hour per month). You are encouraged to complete more than the minimum of 15 hours).
