PROGRESS REPORT/CONTINUING REVIEW APPLICATION is a communication made by the primary investigator/ researcher to update the ethics review committee on how the study is moving.
Through this report, committee will learn not only the progress and accomplishment of the study, but also the following:
Changes in participant population
Withdrawal from study
Changes in recruitment process
Changes in informed consent process
Discovery of latest evidence that is pertinent to note and be included in the concept of the study, that shall be added to literature review
Frequency of report submission will really depend on the amount of the risk that a study will harbor. But generally, all studies MUST SUBMIT THEIR PROGRESS REPORT EVERY THREE (6) MONTHS OR AT INTERVALS APPROPRIATE FOR THE DEGREE OF RISK, DURATION OF THE STUDY PROTOCOL AND SAMPLE SIZE AND TO FILE CONTINUING REVIEW ANNUALLY AS PER RENEWAL OF THE ETHICAL CLEARANCE.

Researchers are required to fully complete the Progress Report / Continuing Review Application Form 3.18.1 ; please follow the instruction below:
All sections of the form must be completed. Do not leave any questions unanswered.
The form must include the electronic signature of the Principal Investigator (PI) and the date of signature.
* Please note: Submissions with incomplete information or missing signatures will not be accepted for review.
