AMENDMENT pertains to any change or revision in the protocol made AFTER ITS APPROVAL.
In any changes for the study (minor or major), the primary investigator / researcher should be mindful in facilitating submission of amendments PRIOR TO ITS IMPLEMENTATION.
Implementing amended activity in a research, without securing an approval from research ethics committee will result to PROTOCOL VIOLATION.Β
Protocol Amendment Submission Guidelines:
Researchers submitting a protocol amendment are required to adhere to the following instructions:
1.π Protocol Amendment Review Form 3.17.1 must be completed and submitted in Word format.
The original formatting of the form must not be modified.
2. The amended research protocol must be submitted in PDF format, along with all relevant supporting documents.
All amendments must be clearly highlighted using bold text and include line numbering.
A footer must appear on every page of the protocol, displaying the version number and version date.
*Please note: Incomplete submissions or those that do not comply with these formatting requirements may result in processing delays.
