Hello Families! We are excited to get started with the school year . In an effort to streamline the back to school paperwork we have made a simple form for you to complete.

The three documents are linked below. Please read these before completing the form. If you have multiple students enrolled at Bagley Elementary School, you will need to complete the form for EACH student.

Also, please complete the Meal Application Form. We are asking each family to complete that form in order to ensure our continued funding.
