Families experience dementia as a whole family*, affecting the routines, fears, hopes, and needs of everyone involved.
The Living Well with Dementia Toolkit helps providers facilitate conversations with families to express their feelings, concerns, and hopes. With this toolkit, we aim to help families build a shared roadmap to gain clarity and support, and to determine the best next steps.
*We use the term "families" to include biological or chosen families, expansive enough to include anyone the person living with dementia deems part of their closest inner circle.
