Science Party Medical Forms

Members of the scientific party should be of good health to participate in the scientific mission aboard Sikuliaq. Your physician can inform you if you are fit for work at sea. 

Sikuliaq subscribes to a shore-side medical service. Medical forms are used to provide patient information to shore-based doctors. 

Please bring sufficient amounts of medications to cover your time at sea and potential delays. It is also recommended that spare glasses and or contacts be brought along.

You will be asked to fill out a medical information form via MFP before you embark.
