Step 1) Complete the AERO Annual Permission Forms (REQUIRED)

You are required to complete the AERO Annual Permission Forms for your student. If you have more than one student, you will need to complete this form below for each student. Please contact us at (708) 496-3330 if you need to request a paper copy of the AERO Annual Permission Forms.

In addition, the Emergency Contact Update Form and Health Update Forms were sent home. If you need a copy or missing a copy of these forms, please ask your Teacher, Program Supervisor, or Principal and they can either email or send copies home with your student.


Step 2) Please remember to re-enroll your student at your resident home school district. (REQUIRED)

Each resident home school district has their own registration process that must be completed before your student attends school. Please select the link below for your resident home school district in which your child attends to complete this process.


If you have any further questions, please contact your student's teacher, program supervisor, principal, or your resident home school district. All forms and registration must be completed before your student attends school.
