Fill out the transcript request form by clicking on the link below or forms are also located in the Registrar’s Office. The charge for each transcript is $5.00. List the name of the school(s) on the form and the registrar will send them to the school/institution for you.
There is a $5.00 charge for each transcript ordered if you e-mail your form to the Registrar you will need to drop off or mail a check to Megan before she will mail your transcripts otherwise complete and print out the form below and drop the form off to Megan with a check and she will gladly process your transcripts.
