Organization name: Life Plan Trust
Purpose/mission: Life Plan Trust collaborates with individuals with disabilities and their families to preserve or enhance their quality of life through future financial planning, trust management services, education, and advocacy.
Population served: Individuals with disabilities and their families
Services/programs/resources offered:
First-Party Pooled Special Needs Trust: Special Needs/Pooled Trusts are an option for individuals with disabilities who have assets of their own (through a settlement, inheritance, back-payment, or other means).
Third-Party Pooled Special Needs Trust: A Third Party Funded Trust is a type of supplemental needs trust, administered by Life Plan Trust that preserves benefit eligibility, while providing for extras (above and beyond basic support).
Contractual Services: Individuals with disabilities or their families are able to contract with Life Plan Trust to provide services, including: visits, advocacy, assistance with money management or other consultative services.
Additional Services:
Serve as trustee
Pay bills on the beneficiary’s behalf
Advocate for appropriate services and assist with ensuring high quality of care
Research available community resources
Teach certain skills, such as money management skills, to individuals with disabilities to increase independence
Life Plan Trust is NOT able to provide residential services or primary supervision.
Costs/requirements to participate:
The Enrollment Fee is currently $900.
The Annual Update Fee is currently $200 and will never increase once a family is enrolled.
Services are currently billed at $85 per service hour.
