Registration Requirements for New & Returning Lakeview Students

The custodial parent/legal guardian must reside at the address that is listed for the student and attend the school in the zone in which the primary custodial parent resides. 

 

  • New & returning student on-line registration updates and required on-line forms must be completed by July 19th (by midnight) through the Skyward Family Access Portal.  Once you have logged on to your Family Access, please be sure and update all of your child’s information and complete all required forms.  If you do not have your Skyward Family Access information please request this in the log on area or contact the school at 615-758-5619. 
  • The Primary custodial parent or legal guardian shall provide two (2) documents that are original (i.e. not a photocopy), recent (dated within the last 30 days ), and include the current name and street address of the parent or legal guardian’s primary residency.     Proofs of residency can be downloaded through Family Access 7/7/17 - 7/19/17 or you may drop them off at the school during the week of July 17th through July 21st. COMPLETING THE REGISTRATION  PROCESS

*** If applicable; students must have on file proof of Custody with the Judge’s signature on the proof (including parents who are divorced or separated).  This should be given to the school any time there is an update through-out the school year.

Acceptable Proofs of Residency for All New and Returning Students:

 The Primary custodial parent or legal guardian shall provide two (2) documents that are original (i.e. not a photocopy), recent (dated within the last 30 days), and include the  current name and street address of the parent or legal guardian’s primary residency.   


  •         Legal document/lease showing proof of residency for specific address (Notary public or typed/hand written letters from the land owner is not accepted)
  •          Mortgage statement with address
  •          Current bill assigned to specific address – electric, gas, water, landline telephone (cell phone bill not accepted).  Must include mailing and service address
  •          Current employment record (example most recent check stub with home address)
  •          Place of registration for voting 
  •          Tennessee Driver’s License with current address
  •          Tennessee ID with current address; or Documentation from Tennessee Department of Human/Children’s Services for benefits with the current address 
  •         Vehicle registration with current address

The Attendance Director may verify a student’s residency at any time during the school year.  During any such investigation the child shall continue to attend school.

 Parents or guardians who use a fraudulent address for enrollment may be subject to restitution to the school district or other costs or fees under Tennessee Law (TCA § 49-6-3003).
