REGISTRATION PACKETS

In order to ensure all paperwork is complete, registration packets should be returned to the District’s Central Registration Office, located inside Wilmington Middle School, to Janene Dunn.  Packets can be picked up at Central Registration. They are also available at WCS Central Office at 341 S Nelson Ave.

MANDATORY KINDERGARTEN ORIENTATION

Mandatory Kindergarten Orientation is available on the following dates and times.  If you cannot attend on the evening that is scheduled for your part of the alphabet, you are welcome to attend the other session. The information provided will be the same in each session.

  • Provide parents with information about a typical school day and the staff with whom their children will work in the upcoming school year.

  • Provide information regarding kindergarten curriculum and other expectations for kindergartners.

  • Provide information regarding Ohio’s Kindergarten Readiness Assessment.

Who is eligible for kindergarten for the 2018-2019 school year?

Children who were born on or before September 30, 2013.

Will I register my student the night of the Kindergarten Orientation?

No, you will register with Janene Dunn at the Central Registration Office, 275 Thorne Avenue.  Please be sure to take care of turning in all paperwork prior to this event.

The following opportunities are available for kindergarten registration at the Central Registration Office, 275 Thorne Avenue:

        Evening Registration, no appointment required:

        Wednesday, April 18, 4:00 – 6:00 p.m.      

        Thursday, May 17, 4:00 – 6:00 p.m.

        Daily Registration

        Please call 937-382-7410 to schedule an appointment for registration, Monday through Friday.

Copies of the following documents are required to register a child in the Wilmington City Schools’ Kindergarten program:

●     Birth Certificate or Passport

●     Social Security Card or copy of 2017 Federal Income Tax return showing child’s Social Security Number

●     Custody Papers (if applicable)

●     Proof of Residency (current utility bill, rental agreement, mortgage closing document – Note: Driver’s License is not acceptable proof of residency).

●     Parent Social Security

●     Parent Driver’s License

●     Immunization Records (copy of immunizations that have been given as of registration day– must also provide complete immunization records which include KDG boosters prior to the first day of school if immunizations not complete at time of registration).

    • 5 doses DPT/DTaP

    • 4 doses Polio

    • 2 doses MMR

    • 3 doses Hepatitis B

    • 2 doses Varicella (chickenpox) or statement from parent or physician indicating date of disease

Central Registration for Wilmington City Schools

Wilmington City Schools Central Registration office registers all new students entering Kindergarten through the Twelfth Grade for the entire Wilmington City School District. The office is located at Wilmington Middle School, 275 Thorne Avenue, in room 139, which is the first door in the hallway to your right as you enter the building.

The office is open for registration Monday through Friday from 8:00 a.m. to 2:00 p.m.  For your convenience, and to minimize waiting time, it is advisable to call (937)382-7410 for an appointment.

General Registration Requirements

The following documentation is required to register your child at Wilmington City Schools:

  • Certificate of Live Birth (NOT hospital record) from county health department or passport
  • Immunization form or records
  • Social Security card or copy of submitted tax form that indicates social security number
  • Proof of Custody (if applicable)
    • Legal documents that show custody or guardianship (official court documents)
    • If custody in process, party has sixty days to complete.  Letter from attorney or
      Court must be presented to show status of case.  If not complete within sixty days,
      Child may be withdrawn. 
  • Proof of residency (lease, rent receipt, utility bill, driver’s license NOT acceptable)
    Affidavit for proof of residency may be required.
  • Name and address of previous school attended
  • Parent/Guardian Driver’s License or State I.D.
  • Parent/Guardian Social Security card
  • Grade card or transcript of previous school attended (required for Grades 9-12)
  • Copy of current I.E.P. (if applicable)
