Congratulations on your employment at West Carroll!

As with all employers, we have legal documents for you to complete. Please follow the links to access the new hire forms that you should complete and bring to the Payroll/Personnel office prior to the first day of employment.    

    



Employment Group Definitions:

Please use the tabs at the top to access your new employee paperwork!

  • Certified Employees - Administrators, Teachers, and Guidance Counselors 
  • Non-Certified Employees - Aides and all Support Staff.
  • Substitute Employees -  Substitutes Only.
  • Short-Term Position - Athletic Coaches, stipend positions, and other temporary employees.
  • Volunteers - Volunteer or Volunteer Coaching positions.
  • Rehired Employees - Anyone rehired that the District Office has existing paperwork for. 


                      Insurance Information ** for those who qualify

All Insurance premiums are paid a month in advance. Adjustments will be made accordingly to reflect any amounts due in payroll for Benefits/Deductions. 

Health insurance for Single is paid for by District 

Dental & Vision are at employee cost

 FULL-TIME EMPLOYEE ONLY -  30+hours to be eligible (PRINCIPAL SEE CONTRACT) 

EFFECTIVE DATE FOR ALL INSURANCES IS THE FIRST DAY ON THE JOB

Life insurance - $25,000 Term Life is paid for by the District (Principal $50,000)

Open Enrollment for all Insurances is held annually in August with a September 1st effective date. 



Please read all directions before completing your new employee forms! 

  • Please print the Live Scan Fingerprinting Form and take it with you to the Carroll County Sheriff's Department located at 301. N. Main Street, Mount Carroll, IL 60153. It is advised that you call ahead to make an appointment.  815-244-9171. There is no payment required.
  • Complete all forms listed in your area of employment.
  • You must bring your original I-9 required identification documents (copies will be made in the District Office).
  • You will have your photo taken and kept in your employee file, for security purposes. 
  • Employees in the Certified, Non-Certified, and Substitute Groups must have a medical examination prior to the first day of employment. 
    Note - Medical examination including TB test must be completed within 90 days prior to employment. -Please set appointment ASAP (if you have had a TB within the last year, include proof of TB results with your physical) The district will reimburse you once the physical has been run through insurance. Then turn in proof of payment.
  • Please print and keep for your records, the Skyward Employee Access instruction sheet.
  • Direct Deposit is required. 
  • Please be aware that this offer of employment is contingent upon receipt and review of the documents and information requested.

Any form with two or more pages may be printed as 2-sided.  You do not need to print the direction pages for any forms.
