Students participating in any athletic sport at Waldron Mercy Academy must complete the proper paperwork prior to the start of their sports season. The following forms can be filled out and submitted digitally. For medical and physical forms, please see below. You may scan and email these forms to pdambrosio@waldronmercy.org, fax them to 610-664-6364 or hand in a physical copy.

Further, all student-athletes must abide by the athletic code of conduct and comply with the Waldron Mercy Academy anti-hazing policy, also found on the WMA website.
