  1. USD 223 School Reach Notification System (Request for Change of Contact Phone Numbers): Enrollment Form for Linn Public Schools: This form should be filled out by new families as they enter our district or when a family changes their phone number(s). Two phone numbers per family is maximum.

  2. Linn Schools Student Medical Information: This is for students in Grades K-5 The form is filled out anually and every student in grades K-5 must have one on file.

  3. Parent E-Mail Letter: This form needs to be filled out if you are enrolling at Linn Schools for the first time or you have changed your e-mail address.

  4. Qualified Admission Form:  All students (Grades 9-12) must have this form on file. Incoming freshman and new high school students must sign and have this form on file. This form is kept on file until the student graduates or is no longer enrolled at Linn High School.

  5. Photo Permission Letter: One per family. Include all students names on one form. Letter stays on file as long as students are enrolled at Linn Public Schools.

  6. Student Internet: Needed for students in grades 4-12. One form for each student. Only needs to be filled out once. Form will be kept on file until student graduates or moves out of district.

  7. Out of District Form: This form must be filled out by all out-of-district families each year. This includes those families who already attend Linn Schools. One form per family is all that is required.

  8. Activity Code of Conduct BOE approved 4-8-13: Must be filled out only once for any child in grades 6-12 who will participate in any activities while a member of Linn Schools. A list of those activities are listed on the back of the form. Any student who does not have this form on file will not be able to participate in the activities listed.

  9. FERPA-Family Educational Rights and Privacy Act: Form must be filled out each year. One form for each child in family.

  10. Enrollment Sign Off: Form must be filled out each year. One form for each child in family.

  11. Meal Payment Information: Information Only--Please Read.

  12. Concussion Form: Must be filled out by all 6-12 students planning on participating in any sport and/or cheer. Form must be filled out each year and must be on file prior to first practice.

  13. Home Language Survey: This form is to be filled out every year.  Fill out one form for each child in your family.
