Registration for 2016-2017 School Year


Returning Students

   The following form can be filled out and submitted electronically before registration.

   4. Dental Exam Form (Kindergarten, 2nd, & 6th Grades)

   5. Physical Exam Form (Pre-K, Kindergarten, 6th, & 9th Grades)


   Registration fee(s) can be paid with a check or cash at registration or through online payment.

   9-12 - Fees are $ 130.00


    All forms are available to be completed and printed at registration if not completed beforehand.

   


 Free & Reduced Lunch Application

If you believe you may be eligible or even if you are uncertain, please complete the application for free/reduced lunch. In addition to free or reduced breakfast and lunch prices, those who qualify have the $95 book rental and $52 milk break fees waived. This information is kept strictly confidential.

 
The application is available to download and print below or will be available to fill out at registration. Please contact
the school office if you have any questions.


New Students & Kindergarten Students

    New students & Kindergarten students must establish guardianship and residency with a district              administrator by providing appropriate documentation. (click here for more information)

 Transportation Information

If you child will ride a bus to or from school, please complete the following information survey. If you are unsure of information (ie. bus stop, bus number, etc.), contact First Student Bus Company at 217-903- 4013 (new phone number as of July 2016).
