Your secure, online health connection

Your health is important to you around the clock—not just during office hours. That's why we have contracted with Martin Health System to offer MyChart. With this Internet connection to both our office AND Martin Health System, you can schedule appointments, request prescription refills, review your health history and more—online, at any time! Please note that this Site is run by Martin Health System. 

With MyChart you will be able to: 
  • Communicate with our practice
  • Access your Laboratory test results
  • Request prescription renewals authorized by your doctor
  • View your recent clinic visits to our practice and Martin Medical Group facilities
  • Schedule annual exams at Treasure Coast Concierge
  • Request appointments at Treasure Coast Concierge
Urgent Medical Matters

Please do not use MyChart to send any messages requiring urgent attention. For emergencies, dial 911. For urgent medical matters, contact Dr. Kardos or Dr. Robbins directly.

How to Access MyChart

1. During your visit to our office, you will receive a letter (i.e. an After-Visit Summary) that has a 10-character access code on it. Find that code.

2. Go to the following website: https://mychart.martinhealth.org/mychart/ 

3. To set up your username and password for the first time, click on the blue "New User? Sign Up Now" button below the grey "Sign In" box. You will see the "Please Identify Yourself" page. 

4. Enter your MyChart Activation Code exactly as it appears in your letter in the "MyChart Activation Code" field, the last four digits of your Social Security Number (xxxx), and your Date of Birth (mm/dd/yyyy) as indicated and click the "Next" button. 

5. Set up your ID and Password:
    • Create a MyChart ID. This will be your MyChart login ID which cannot be changed. Make sure to choose a name that is secure and easy to remember. 
    • Create a MyChart password by entering it in both the "Password" and "Retype Password" fields. You can change your password at any time. 
    • Select a security question from the drop down menu and enter a Secret Answer. This can be used at a later time if you forget your password. 
    • Click "Next".
6. When new information such as results or a message is available in your MyChart account, you will receive email notification at the email address entered on this page. (Note: You are not required to maintain an email address.)

7. Select the "Yes" button under "Enable E-mail Notification".

8. Enter your E-mail address in both fields.

9. Click sign-on.

10. Click "Accept" to accept the MyChart legal terms and conditions.
