We encourage you to participate in the Ages and Stages Questionnaire (ASQ-3) for your children between the ages of 0 and 5. This tool is available for use by all parents in the Walled Lake School district. Your child does not have to be enrolled in the district preschool programs to use this helpful assessment.

Ages and Stages Questionnaire
Ages and Stages (ASQ-3) is a screening tool that will provide a quick check of your child's development. The ASQ-3 looks at five developmental areas for your child from birth to five years old. Have fun completing this questionnaire with your child. Make sure your child is rested, fed and ready to play before you try the activities.

La serie de cuestionarios Edades y Etapas Tercera Edición (ASQ-3) es una herramienta de evaluación que le permitirá vigilar el desarrollo de su niño/a de manera sencilla y rápida. La primera parte del cuestionario ASQ-3 monitorea cinco áreas de desarrollo infantil.La sección titulada Observaciones generales contiene preguntas importantes sobre el desarrollo de su niño/a y le ofrece a usted la oportunidad de expresar cualquier inquietud o duda que tenga acerca del mismo.Disfrute el proceso de completar este cuestionario con su niño/a, y asegúrese de que él/ella haya descansado y comido y que esté listo/a para jugar antes de empezar a hacer las actividades.
