Health Requirements

Students entering any Illinois public or private school (including charter schools) must provide proof of the following forms, approved by both the Illinois Department of Public Health and the Illinois State Board of Education.

School physical forms and immunizations are due on or before September 15th or 30 days from the first day of enrollment. The exam must be completed no earlier than a year (365 days) prior to the start of attendance at an Illinois school. We are unable to accept out-of-state forms. Failure to submit a physical and/or immunizations will result in exclusion from school. It is imperative to contact the school nurse with any questions or concerns. Dental exams are due May 15th (of the current school year). See chart below for health requirements by grade. See forms.
Vision exams are due September 15th or 30 days from the first day of enrollment. See chart below for health requirements by grade. See forms.
Health concerns forms are due annually, at the beginning of the school year. The forms are an important in keeping the school nurse and teachers informed of any health-related concerns or conditions that may impact a child's school day. See forms.

Exemptions and Objections

Religious Objection: Parents who object for religious reasons to their child being immunized must submit a Certificate of Religious Exemption form, which must be signed by a healthcare provider.

Physician Exemption or Objection: Currently there are two avenues by which a physician may exempt a child from a required immunization (see School Health Code for more Information)

  1. Physician Statement of Immunity (subject to approval by the Illinois Department of Public Health)
  2. Medical Exemption a signed physician's statement of the medical reason the immunization is unsuitable.
