Health Requirements
Students entering any Illinois public or private school (including charter schools) must provide proof of the following forms, approved by both the Illinois Department of Public Health and the Illinois State Board of Education.
Vaccine Preventable Diseases
Immunization protects against the following 10 serious diseases, which can cause disability and death. These diseases used to strike thousands of children each year. Today there are relatively few cases, but outbreaks still occur each year because some babies are not immunized. Exemptions and Objections
Religious Objection: Parents who object for religious reasons to their child being immunized must submit a Certificate of Religious Exemption form, which must be signed by a healthcare provider.
Physician Exemption or Objection: Currently there are two avenues by which a physician may exempt a child from a required immunization (see School Health Code for more Information)
- Physician Statement of Immunity (subject to approval by the Illinois Department of Public Health)
- Medical Exemption a signed physician's statement of the medical reason the immunization is unsuitable.
