Health Requirements

Students entering kindergarten (or first grade if not previously enrolled in kindergarten), sixth grade and ninth grade at any Illinois public or private school (including charter schools) must provide proof of physical examination on a form approved by both the Illinois Department of Public Health and the Illinois State Board of Education. Students entering an Illinois school from out of state or out of country for the first time at any grade level must also provide the physical exam form. The exam must be completed no earlier than a year (365 days) prior to the start of attendance at an Illinois school.  Illinois School Health Code
  • Physicals, immunizations, vision exams are due on or before September 15th or 30 days from the first day of enrollment.  See chart, below.
  • Dental exams are due May 15th (of the current school year).  See chart, below.
Failure to submit a physical and/or immunizations will result in exclusion from school until the documents are received.

EXEMPTIONS AND OBJECTIONS:

Religious Objection: Parents who object for religious reasons to their child being immunized must submit a Certificate of Religious Exemption form, which must be signed by a healthcare provider.

Physician Exemption or Objection: Currently there are two avenues by which a physician may exempt a child from a required immunization (see School Health Code for more Information)

  2. Medical Objection Note: not all immunizations are included in immunity statements from the physician
