NEW: Beginning with the 2017-2018 school year
, an age-appropriate developmental screening and an age-appropriate social and emotional screening are required parts of each health examination. If proof of the required screenings are not provided, qualified school support personnel may, with a parent/guardian’s consent, offer the screenings to the child.

Health Requirements - Forms

Religious Objection: Parents who object for religious reasons to their child being immunized must submit a Certificate of Religious Exemption form, which must be signed by a healthcare provider.

Physician Exemption or Objection: Currently there are two avenues by which a physician may exempt a child from a required immunization (see School Health Code for more Information)
  1. Physician Statement of Immunity
  2. Medical Objection Note: not all immunizations are included in immunity statements from the physician
