- In accordance with N.J.S.A. 18A: 40-42, the New Jersey Department of Health and Senior Services has developed educational facts about the Human Papillomavirus (HPV). We are required by the State Education Department to distribute this information for your review. Information re: the HPV vaccine can be accessed from the following link: HPV Info Please discuss this with your child’s healthcare provider for further information.
- In accordance with N.J.S.A. 18A:40-21.2, the New Jersey Department of Health and Senior Services requires public schools to distribute this fact sheet annually to the parents or guardians of students in the 6th grade. - If OVER THE COUNTER MEDICATIONS are to be given, please fill out this form and return as soon as possible. OTC medications WILL NOT be given without the completion of this form. This is in accordance with the NJ STATE LAW.
