The "Equipment and Help Request" form is for requesting equipment borrowing and/or setup (for example, borrowing a video camera), or staff assistance (for example, setting up a microphone) at live classes or events.
The "Request Group Training or One-on-one Appointment" form is for requesting a help appointment (for example, to help you learn how to use a handheld audio recorder and transfer recordings to your laptop) or a group training session (for example, on the basics of using a SMART Board) with a staffperson from Media Services.
The "Report an Issue" form is for reporting a problem with audio-visual equipment (for example, a flickering projector).
The "Feedback" form is for sharing your kudos and ideas for improvement regarding audio-visual equipment, services, and staff.
