Registration and Forms

Registration for the St. Hugo Science Fair is now open!

November 27-December 13, 2017
 

The registration process for the St. Hugo Science Fair has changed this year. 


There is a three-part registration process.  All students must complete all three steps.


Step 1. Complete the  On-line Registration Form.  An email confirmation will be sent to the Parent #1 email address. 


Step 2. Registration Fee: Due December 15, 2017 Registration fee ($20.00) per participant).     Please make checks payable to "St Hugo School".       In the memo- write "Science Exchange" and turn in to your classroom teacher and they will provide you with the required Proposed Experiment Form. 

This form is also available to download from the files below.


Step 3. Complete and turn in the  Proposed Experiment Form and download any other needed Supplemental forms (see below).

    • Additional forms that may be required in this step vary with the nature of the student’s project. Examples: Human Subjects, Vertebrate Animals, Hazardous substances, etc. Make sure you have all required forms needed along with your Proposed Experiment Form.
    • ***** The Wake up to Science Booklet is now available to download from on the website- please note: the booklet is broken down into 3 parts (see files below).

Notes:
  • All students must register On-line by December 13, 2017 
  • The Proposed Experiment Form is required to be turned into your child's science teacher for approval by January 8, 2018
