Forms Information

• Parent Permission Form & Student-Athlete Contract: must be signed and returned for every new sports season.
• Concussion Acknowledgement & Signature Form: must be signed and returned for the first sport of year played. This stays on file for all other sports played that school year.
• Authorization For Medical Attention and Informed Consent: this can be submitted with the first sport played. It will carry to other sports later in year. If there are any changes during course of year please update.
• Preparticipation Physical Evaluation: This form should be filled out and taken with athlete when reporting to doctor for a physical.
• Well Exam: This form should be filled out by the doctor preforming the physical. All physicals are considered current for a two-year cycle. Meaning physicals must be renewed within two years to the date of the last one.

Below each form is a link to print any forms your athlete needs.

NO athlete will be allowed to start any new season, first day of practice, without all forms in filled out, signed and returned to the Athletic Director.
